Telephone Connect is a program that provides wellness checks over the phone to local homebound seniors, ensuring their safety and well-being. A trained volunteer will provide wellness checks by phone call on a regularly scheduled basis to seniors who are in need of someone to touch base with them. Volunteers, who are interviewed, screened, and trained, place calls to folks at agreed upon times.
- Telephone Connect offers peace of mind to folks, family and friends, as well as daily socialization.
- Telephone Connect is not a referral service or medical alert service.
- Individuals may self-refer or be referred by a caregiver, family member, or service provider.
Telephone Connect is intended to serve town of Hartford seniors. This may be temporary while recuperating from an illness or it may be a permanent situation. Married couples who are homebound are also eligible for Telephone Connect.
